Completed application packet must include:

Letter of application*
A completed certificated application form
Resume
Affirmative Action Sheet (Optional)
Professional Reference Forms/Letters of Recommendation
Copy of teaching certificate
Copies of all college/university transcripts*
A college/university placement file*
Copy of Praxis II test results
Release Form SPI 1588

*Items not required if apply as certificated substitute teacher
